  • Other Purpose

Explain why you have a "reasonable necessity" to build the Title II Firearm.

  • Are you: building a new firearm from scratch, or are you modifying an existing firearm?

    For Example:

If you are making a Silencer or finishing an 80% complete Lower Receiver, you are building a new firearm
If you are converting a rifle into an SBR, you are modifying an existing firearm

  • Are you fixing or restoring a broken firearm that was previously registered as an NFA Firearm?

  • In what Country was your Firearm Manufactured?

  • Manufacturer Information

Because your Firearm was manufactured in a different country, the ATF wants two different addresses:

          1.  The Name and Address of the company that manufactured your firearm, and
          2.  The Name and Address of the company that brought your Firearm into the United States (the 'importer').

  • What is the Manufacturer's Name   Address?

Manufacturer: Fabbrica d'Armi Pietro Beretta S.p.A.
    City: Gardone Val Trompia

  • What is the Model of the Firearm?

If you choose to give your new firearm a Model Name, the Model Name Must be engraved on the Lower Receiver or Silencer Tube exactly as it is entered in this field
  • What is the Model of the Firearm?

    For Example:
If you are converting your Colt M4 Carbine into an SBR you would enter M4 Carbine in this field
The Model Name Must be entered exactly as it appears on the NFA Firearm

  • Directions on entering the Caliber or Gauge of the Firearm

    Notes:
    1. You are only allowed to enter 1 caliber.
    2. Do not enter "Multi" or "Multi-cal" - If you enter anything other than a single specific caliber, YOUR APPLICATION WILL BE DENIED!
    3. Because of the limited space available on the Form 1, we suggest using the following abbreviations:
          cal for caliber
          mm for millimeter
          ga for gauge,
    For example:

          .223 cal       9 mm       12 ga

  • Directions on entering the Caliber or Gauge of the Firearm
